ABSTRACT
Primary CL / P repairs, revisions, and secondary procedures – cleft nasal surgery, speech surgery, and alveolar bone grafting (ABG) – performed from 2014 to 2018 were identified from the Pediatric National Surgical Quality Improvement Program (NSQIP) database. The usage estimates were derived using univariable and multivariable logistic regression. A Kruskal-Wallis rank sum test and multivariable linear regression were used to assess the differences in timing for each procedural cohort.
The primary outcome measures were the probability that a patient could be assigned to a certain race / ethnic group and the age at which patients of different races / ethnic groups were operated on.
23,780 procedures were analyzed. After checking gender, diagnosis and functional status, there were significant differences in the usage estimates between the procedural groups. The main use was in black patients for rhinoplasty (OR = 0.70, P = 0.023), ABG (OR = 0.44, P <0.001), and speech surgery (OR = 0.57, P = 0.012) lowest, and highest in patients who were Asian patients in all operation cohorts (OR 2.05-4.43). The timing of the surgery also varied depending on the breed, although the differences were minimal.
CONCLUSIONS: Estimates of use and timing of secondary cleavage procedures varied by ethnicity, particularly in black patients (poor use) or Asian patients (high use). Further studies should determine the causes and effects of an inadequately used and / or delayed gap treatment.
